Role overview

The purpose of the Registered Nurse is to deliver safe, high‑quality clinical care to clients receiving Support at Home, NDIS, and aged‑care services, ensuring their health, dignity, independence, and wellbeing are maintained in accordance with professional nursing standards and organisational care frameworks. The role provides specialised nursing interventions, supports care plans, collaborates with multidisciplinary teams, and upholds strict clinical governance to ensure every client receives personalised, compassionate, and compliant healthcare.

Key responsibilities
  • Deliver safe, high‑quality nursing care to clients in home‑care, disability, and community settings.
  • Conduct comprehensive health assessments, monitor vital signs, and identify changes in client conditions.
  • Administer medications, injections, and treatments in line with clinical guidelines and AHPRA standards.
  • Provide wound care, chronic disease support, and other essential nursing interventions.
  • Develop, implement, and review individualised care plans tailored to client needs and goals.
  • Maintain accurate clinical documentation, progress notes, and incident reports.
  • Educate clients and families on health management, medication use, and lifestyle strategies.
  • Collaborate with GPs, allied health professionals, and support workers to ensure coordinated care.
  • Uphold infection‑control practices and contribute to My Help’s clinical governance framework.
  • Respond promptly to deterioration, risks, and emergencies, escalating to medical practitioners when required.
Qualifications and skills
  • Current AHPRA registration as a Registered Nurse (Division 1).
  • Bachelor of Nursing or equivalent AQF Level 7 qualification.
  • 1–2 years of nursing experience in aged care, disability, community care, or hospital settings (new graduates also considered).
  • Strong clinical skills in assessment, wound care, medication administration, and chronic disease management.
  • Knowledge of NDIS Practice Standards, Support at Home requirements, and person‑centred care models.
  • Current First Aid and CPR certification.
